About the Lead Patient Care Coordinator Role
Upperline Health is seeking an energetic, compassionate, and driven Lead Patient Care Coordinator who will be responsible for our Madison, AL office. The Lead Patient Care Coordinator will assist our physicians in delivering comprehensive health services in the clinic setting and will lead the clinic with a range of clinical and administrative tasks. Candidates must be flexible problem solvers and collaborative team players who are eager to take ownership of patient care and the success of the clinic. Ideal candidates will have podiatry or orthopedic understanding.
Lead Patient Care Coordinator Responsibilities
- All the responsibilities outlined in the patient care coordinator role.
- Deep understanding of Upperline’s comprehensive care program, Upperline Plus, and responsible for patient enrollment and engagement efforts in collaboration with broader care team
- Responsible for daily coverage in clinic and working with other Clinic Leads in market to staff all clinics appropriately.
- Responsible for recruiting and hiring of non-provider clinical roles, performance management, employee and doctor experience, new employee on boarding and minimizing turnover.
- Accountable for clinic key performance indicators – clinical, operational, and financial – in collaboration with Regional Manager.
- Facilitate clinic daily huddles with all members of the clinic team each day.
- Responsible for clinic referral conversion, monitoring of physician schedule templates, and adherence to scheduling cadence for high-risk patients.
- Maintain a strong positive culture in the clinic; build trust and respect within the team.
- Assist regional leadership with implementing and maintaining key corporate initiatives and objectives within the clinic setting.
- Perform monthly audits of the clinic operations as well as the EMR documentation.
- Accountable for the clinic patient experience measured by net promoter score.
- Responsible for clinic and equipment licensure.
- Travel to surrounding area clinics to provide support if needed.
- Make timely and high-quality decisions.
Lead Patient Care Coordinator Qualifications
- Previous experience as a Patient Care Coordinator.
- Podiatry or Orthopedic clinic experience preferred.
- Excellent computer skills and strong knowledge of medical terminology
- Previous experience working in an EMR – preferably athenahealth EMR.
- Must be an effective communicator with excellent grammar and interpersonal skills.
- Exceptional customer service orientation featuring an empathetic, compassionate, and professional demeanor with each interaction.
- Demonstrated experience as a motivated and dedicated team leader with a stable work history.
- Reliable transportation and the ability to pass a background check and drug test is required.
Physical Demands and Working Conditions:
- Day-to-day work includes desk computer work and direct patient interaction, facility staff and physicians.
- Position may have extended periods of standing and to lift more than 5 lbs.
- The work environment is characteristic of a health care facility with air temperature control and moderate noise level.
Compensation:
- Compensation is commensurate to compensation for similar positions in the region and based on prior training and experience.
