About the Nurse Practitioner (APP) Position
Upperline Health is seeking a Nurse Practitioner to provide clinic-based endocrinology care primarily in our {location}. In collaboration with Upperline’s local Physician team, APPs develop complex plans of care in accordance with the patients' health status and overall goals and values; provide medical diagnostics and treatments in the clinic, in the patients home, or via telehealth, consistent with the APP’s scope of practice and the patient's medical condition; educate patients and families on medication usage, diabetes technology (as applicable), lifestyle modification, diet & nutrition, and crisis anticipation & prevention. They will also communicate medical information to patient's physicians, other providers and Upperline’s Physician team and broader interdisciplinary team. Ideal candidates will have experience with the following: Endocrinology and/or Primary Care/Internal Medicine with a strong focus in diabetes management, thyroid disease, osteoporosis, and metabolic health; experience adjusting insulin and non-insulin regimens; and comfort interpreting common endocrine labs and diagnostic studies. Most importantly, candidates must demonstrate compassion for patient needs, have great attention to detail and thrive in a collaborative environment with rapid growth. What You'll Do:
• Develop and implement clinical plans of care for adult patients with endocrine and metabolic conditions (e.g., type 1 & type 2 diabetes, thyroid disorders, obesity, osteoporosis, adrenal and pituitary disorders).
• Gather patient history, perform focused physical exams, order/interpret relevant diagnostics (e.g., A1c, CMP, lipid panels, thyroid function tests, microalbumin urine, endocrine dynamic labs as applicable) in clinic, via telehealth, or within the patient's home when appropriate.
• Initiate and titrate diabetes and metabolic therapies, including insulin, GLP-1/GIP agents, SGLT2 inhibitors, and other endocrine medications consistent with scope and protocols.
• Review and interpret glucose data from SMBG/CGM downloads; adjust regimens and provide coaching to improve time-in- range and reduce hypoglycemia risk.
• Support diabetes technology as applicable (e.g., CGM initiation/education, insulin pump support in collaboration with supervising physician and device educators).
• Provide patient education on medication usage, side effects, sick day management, hypoglycemia recognition/treatment, diet & nutrition, physical activity, and behavior change strategies.
• Coordinate preventive care and complication screening in collaboration with the interdisciplinary team (e.g., eye exams, foot care coordination, renal monitoring, cardiovascular risk reduction).
• Collaborate with the interdisciplinary team to meet the patient and families physical as well as psychosocial needs, including referral to nutrition, pharmacy, and community resources.
• Communicate with other Upperline clinical team members and patients' community providers to coordinate optimal care and resources.
• Communicate with Upperline's local physician to review patient records and care plans on a timely basis consistent with state regulations and company standards and policy.
• Maintain patient medical records and medical documentation consistent with state regulations and company standards and policy.
• Complete accurate and timely documentation according to risk adjustment standards per education provided.
• Take part in weekly interdisciplinary team meetings and scheduled education sessions.
• Provide telephonic support for escalated calls from triage nurse.
• Participate in continuing education as required by the state Board of Nursing.
• Prescribe medication as permitted by the state Board of Nursing.
• Other related duties as deemed necessary.

Required Skills, Education, and Competencies:
• Master’s degree in nursing required
• Board Certification as an Adult, Adult-Geriatric or Family Nurse Practitioner required
• Current State Registered Nurse & Nurse Practitioner license required
• Basic Life Support (BLS) certification is required
• 6-months of nurse practitioner experience in Primary Care/Internal Medicine or Endocrinology
• Endocrinology experience preferred; strong diabetes management/Metabolic Disease experience highly preferred
• Demonstrated ability to interpret and act on endocrine labs (e.g., A1c, thyroid studies, calcium/vitamin D/PTH, lipid management) and to develop treatment plans
• Experience with diabetes technology (CGM; insulin pump familiarity) and data review (downloads/reports) preferred
• Knowledge of insulin titration, hypoglycemia management, and sick-day rules; ability to provide high-quality patient education
• Value-based care experience preferred
• Possession of DEA registration or eligibility
• Proficiency using Microsoft Office products
• Excellent oral and written communication skills to ensure that each of your
interactions with patients, family caregivers, clinicians, and team members is
effective and meaningful.

Compensation
Compensation is commensurate to compensation for similar positions in the region and
based on prior training and experience.
