ABOUT THE PRIMARY CARE MEDICAL ASSISTANT
Upperline Health Plus is seeking a Primary Care Medical Assistant who delivers high-quality, patient-centered care in a value-based practice.
In our patient-centered and outcomes-driven environment, the Medical Assistant supports patients in the clinic by rooming patients, obtaining medical history, and taking vital signs, while also ensuring care beyond the visit through proactive outreach and care coordination.
The Medical Assistant provides direct support to the Upperline Plus Primary Care Provider to ensure efficient clinic operations and effective team-based care.
The ideal candidate is organized, patient-focused, and comfortable working at the top of their scope in a collaborative care model. Compassion, attention to detail, and strong teamwork skills are essential.

WHAT YOU’LL DO
- Room patients; obtain and document vital signs, medical history, and chief complaints
- Assist providers with in-clinic visits and health assessments
- Support virtual provider encounters from the clinic setting
- Accurately document clinical data in the electronic health record (EHR)
- Perform medication reconciliation and identify discrepancies
- Conduct telephonic outreach to coordinate preventive and ongoing care
- Manage in-basket messages, prescription refills, and prior authorizations
- Support referrals, scheduling, records, and DME coordination
- Assist patients with accessing community resources
- Prepare and maintain clinical equipment and supplies
- Collaborate with interdisciplinary team members
- Participate in team huddles and meetings
- Perform clinical tasks within scope (injections, phlebotomy, POCT)
- Perform other duties as assigned
QUALIFICATIONS OF THE PRIMARY CARE MEDICAL ASSISTANT
- Certified Medical Assistant or minimum of two years’ experience in primary care
- Primary care or outpatient clinic experience strongly preferred
- MA or phlebotomy certification strongly preferred
- Strong knowledge of medical terminology and basic pharmacology
- Experience with team-based care models
- Care coordination or case management experience preferred
- Experience with adult and geriatric patients preferred
- EMR experience preferred; AthenaHealth ideal
- Proficiency in Microsoft Office tools
- Reliable transportation and valid driver’s license
- Strong customer service orientation and communication skills
- Ability to multitask in a fast-paced environment
COMPENSATION
Compensation is competitive and commensurate with similar roles in the region, based on prior training and experience.
