The Director of Population Health plays a key leadership role in advancing Upperline Health’s value-based care strategy. This individual will lead the implementation and
ongoing improvement of field-based population health initiatives designed to improve patient outcomes, enhance quality performance, and optimize total cost of care.
Working closely with clinical leadership, care teams, operations, and analytics partners, and reporting to the Senior Vice President of Market Operations, the Director will help translate organizational priorities into actionable programs that improve care delivery across Upperline’s markets. This role will focus on aligning clinical teams, operational workflows, and data-driven insights to ensure patients receive proactive, coordinated, and high-quality care.

The Director will also serve as a key connector across teams, ensuring that population health programs are effectively implemented and that care teams have the tools, data, and support needed to succeed.

Core Responsibilities
• Lead the implementation and continuous improvement of population health programs that support Upperline’s value-based care model.
• Identify opportunities to improve patient outcomes, quality performance, and cost of care through targeted interventions.
• Develop initiatives focused on key population health priorities such as screenings, clinical documentation, transitions of care, chronic disease management,
medication adherence, preventive care, and patient engagement.
• Partner with leadership to translate strategic priorities into operational programs and workflows.
• Work closely with physicians, advanced practice providers, care navigators, pharmacists, and other care team members to support program implementation.
• Build strong relationships with employed providers to drive engagement and adoption of population health initiatives.
• Monitor and analyze population health performance across quality, utilization, and financial metrics.
• Partner with analytics teams to develop reporting, dashboards, and insights that guide program improvement.
• Use data to identify opportunities, prioritize interventions, and evaluate program effectiveness.
• Provide guidance and support to care teams as they adopt new processes and tools.
• Participate in strategic planning and cross-functional projects as needed.
• Perform other duties as assigned.

Qualifications
• Bachelor’s degree required
• Advanced degree preferred (MPH, MBA, MHA, or related field)
• 3+ years of experience in management consulting, investment banking, or healthcare operations
• Experience in healthcare operations, population health, or value-based care preferred
• Strong leadership and stakeholder engagement skills
• Demonstrated ability to influence across cross-functional teams
• Demonstrated ability to single-handily analyze an opportunity and execute an operational improvement initiative against that initiative
• Highly organized with strong project management capabilities
• Data-driven mindset with strong analytical skills
• Ability to develop business cases, ROI analyses, and performance improvement strategies
• Strong communication and collaboration skills
• Comfort operating in fast-paced, evolving work environment
