About the Pharmacist (PRN) Position
Upperline is looking for a motivated PRN Pharmacist to support Medicare beneficiaries in our accountable care organization.
This role will be critical in providing a superior level of care to patients while lowering overall cost for the healthcare system. To be successful at this position, you need to be organized, self-motivated, and personable. You will perform interactive, patient-centered, person-to-person, telephonic comprehensive medication reviews with beneficiaries and evaluate the profiles of those beneficiaries to determine if changes are warranted to optimize the drug therapy and minimize the potential for adverse drug reactions. This role will contribute to our long-term success by delivering a critical layer of support to our beneficiaries. Upperline is looking for strong performers who are comfortable in an entrepreneurial, fast-paced environment, who can handle tasks both big and small and who can effectively work with a diversity of stakeholders (including doctors, other members of Upperline’s care team, and operators). The Clinical Pharmacist role is a critical member of the care team. This position is in-person at our Ascend Pharmacy location (447 Metroplex Dr, Nashville, TN 37211)
We are a closed-door pharmacy with a Monday-Thursday 8am-4:30pm and Friday 8am-3pm schedule.
What You’ll Do:
- Develop and implement quality improvement initiatives related to medication management
- Deliver MTM services, including but not limited to, comprehensive medication reviews, disease specific medication reviews, and targeted interventions to assess appropriateness of all medications, identify barriers to adherence, optimize medication therapy and to improve clinical outcomes
- Assist with managing medication costs through cost-effective interventions which may include, but are not limited to, formulary or therapeutic interchanges, pill splitting opportunities, combination products and assessing unnecessary and/or duplicate medication therapies
- Efficiently assess prescribed medication therapy for unnecessary medications, need for additional medications, patient educational gaps, ineffective medications, inappropriate doses, adverse drug reactions, adherence concerns and opportunities for medication synchronization.
- Communicate professionally and discuss/document clinical recommendations/interventions to prescribers and interdisciplinary care teams
- Follow-up with patients and/or caregivers with an additional follow-up phone call subsequent to the initial in-person visit and appropriately document outcomes in a timely manner
- Ensure patients and family members have a positive experience by providing a high level of customer service by always responding promptly, professionally, and politely
Qualifications of the Clinical Pharmacist
- Doctor of Pharmacy degree - PharmD is mandatory
- Completion of an accredited PGY1 and/or PGY2 pharmacy residency program preferred
- Medication Therapy Management (MTM) training certificate from a recognized/accredited program OR 3 years of experience in delivering MTM services directly to patients strongly preferred
- Certified diabetes educator is a plus
- Excellent listening skills; present a warm, friendly, and empathetic demeanor demonstrating active listening techniques
- Consistent professional demeanor; maintain professional attitude and behavior at all times
- Excellent interpersonal skills; demonstrate empathy with patients and family members in tone, language, questions and demonstrate patience, caring, sensitivity, empathy, and cultural competency throughout job responsibilities
- Conflict resolution skills; able to defuse potential customer or patient conflict utilizing the appropriate tone, support, questions or counsel and respond in a supportive manner
- Strong work values; able to follow all company and departmental processes and procedures
- Self-management skills; self-motivated, organized, and detail oriented
Computer/Technology Competency:
- Proficient in use of Microsoft Word, Excel, and Outlook
- Able to multi-task using multiple computer databases/platforms/application

Compensation is commensurate to compensation for similar positions in the region based on prior training and experience.
